| Patient Referral Process
The method for referring a patient to a specialist is as follows:
- Medical Home doctor determines a specialist’s
care is needed.
- They call SharedCare for referral.
- A SharedCare caseworker selects next eligible specialist.
- They call the specialist’s office
for an appointment.
- SharedCare notifies the patient of their appointment.
- The specialist’s office receives
written notice -- faxed or hand carried by patient confirming
the appointment.
- When the patient arrives, the present their SharedCare eligibility
card.
- No additional or prior authorization from SharedCare is required.
Your office staff should:
- Check the card’s expiration date.
- Bill any charges on a Health Insurance Claim form to SharedCare.
- If SharedCare does NOT receive a claim form, your practitioner
will NOT receive credit toward the 6 annual illnesses.
- NOT expect payment when you bill.
- If you want to collect a nominal payment,
- Collect it directly from the patient at the time of the visit.
- Indicate receipt of this on your Claim Form
